A practical guide to Continuous Care

How Continuous Care works between visits

Continuous Care brings Chronic Care Management, Psychological Screening, and Remote Therapeutic Monitoring into one coordinated workflow. The questions below explain who qualifies, what patients experience, how documentation is created, and where the practice remains in control.

The three programs address different parts of care, but they share the same operating idea: maintain structured contact between appointments, surface meaningful changes early, and return organized documentation to the clinical team for review.

Question 01

Which patients qualify for Chronic Care Management (CCM)?

CCM applies to Medicare patients with two or more chronic conditions expected to last at least 12 months or until death. In specialty practices, this includes patients with chronic pain, post-surgical chronic conditions, depression, anxiety, or other ongoing comorbidities. Not every patient qualifies: a single isolated procedure without chronic comorbidities, such as an isolated elbow or shoulder case, typically does not meet eligibility. Populate screens your existing panel and identifies eligible patients before enrollment. CCM is available for pain management and orthopedic practices.

Question 02

Which patients qualify for Remote Therapeutic Monitoring (RTM)?

Patients with qualifying musculoskeletal conditions are identified automatically from diagnosis codes in the visit note. RTM is available for podiatry, pain management, and orthopedic practices.

Question 03

How does Psych Screening work?

Populate's TEMPO program delivers behavioral health screenings automatically between visits via voice or SMS, so no in-office forms are required. Assessments cover depression (PHQ-9), anxiety (GAD-7), somatic symptoms, substance use, ADHD, bipolar features, and suicidal ideation. If a patient screens positive in any domain, the system automatically triggers a full instrument on a follow-up call. Scored results, ICD-10 codes, and a billing-ready SOAP note are generated for your review. Psych Screening is available for podiatry, pain management, and orthopedic practices.

Question 04

How does RTM work?

The provider enrolls the patient from the visit note, and the patient completes setup through a secure Patient Portal link. Check-in surveys go out on the 1st and 15th of each month, and you get an alert when responses cross a threshold you set.

Question 05

How does CCM work?

The provider enrolls the patient from the visit note, and the patient signs the Continuous Care consent. Rachel, our AI care assistant, then calls the patient on a fixed monthly schedule to collect updates on symptoms, medications, and their care plan. Every call is logged on your CCM dashboard with a recording, and the monthly note and charge are created once 20 minutes of care are completed.

Question 06

How does billing support work?

Populate generates billing-ready documentation for Chronic Care Management, psychological screening, and Remote Therapeutic Monitoring, with no manual charting required. For CCM, completed monthly calls auto-insert CPT 99490 once the 20-minute threshold is met. For psych screening, CPT codes 96127, 96138/96139, and 96130/96131 are assigned based on the phase of the protocol completed. For RTM, Populate prepares documentation and proposed codes based on the documented setup, monitoring days, and treatment-management time; the applicable code set may include 98975, 98977 or 98985, and 98979, 98980, or 98981. Your team reviews, confirms codes, and submits. Populate handles the documentation, while you retain billing control.

Question 07

Does this replace staff entirely?

No. CMS requires the billing practitioner to retain clinical oversight and reassessment responsibility, so CCM and RTM cannot be fully delegated to a vendor. Populate handles between-visit outreach, structured check-ins, escalation routing, and documentation drafts. Your team reviews flagged patients, confirms medical necessity for RTM, and provides physician sign-off where required. Most practices find the incremental staff time is minimal once the workflow is set up.

Question 08

Is this HIPAA-compliant?

Yes. Populate Continuous Care is built on the Populate platform and is designed to meet HIPAA requirements for protected health information.
